Community Partner Certification Form for 2013 funding

United Way has identified and is working toward community goals. In doing this, United Way also has a responsibility to assure the community that their resources are directed towards organizations that meet high standards.

To that end, we are providing all organizations who would like to be considered eligible for funding the opportunity to agree to our standards in advance.  While the standards are not new, this process is new as of 2011 for United Way of Douglas County.  This certification is in no way a guarantee of funding, but it does provide the vehicle for an organization to be eligible for funding.

Any organization is invited to complete the downloadable certification form if it is appropriate.  As you know, this planning work is time consuming and collaborative efforts can be challenging, but United Way hopes that anyone who is motivated to help will take these steps towards certification.  If you are not interested in being eligible for funding, you need not complete this form.

The deadline for submitting the completed Community Partner Certification Form is May 15, 2012.  While the form is not challenging to complete, it is important for any organization’s staff and volunteer leadership to carefully determine whether or not to seek United Way Certification.

Education Community Impact Process 2012-2014

Community Partners have submitted collaborative funding proposals in the area of Education.  All proposals are focused on ensuring that Students are ready (prepared and supported) for school every day.  To that end, approved programs target one of four community indicators.  Community Indicators are based on transition years:

Kindergarten – Dynamic Indicators of Basic Early Literacy (DIBELS Next) assessment identifies the percentage of students who are performing at the expected level (benchmark) to be on-grade readers in 3rd grade. 

Grade 5 – County-wide academic proficiency as demonstrated by 5th grade reading proficiency and 5th grade math proficiency

Grade 8 & 9 -- Attendance (collaborative program still in development)

Grade 12 – Students graduating with their class (collaborative program still in development)

Self Sufficiency Community Impact Process 2013-2015

Certified Community Partners are working together to develop collaborative funding proposals that target the goal: People have steady jobs and financial stability.  To that end, proposals are being developed to target community indicators that promote financial independence and economic security for individuals, emabling to meet their basic needs for living, maintain employment, respond to emergencies, maiintain affordable housing and build personal financial assets.

Health Goal Funding Application

Certified Community Partners are eligible to submit a program funding application to United Way for any programs that target the goal: People of all ages have access to mental and physical healthcare. 

Health Goal proposal forms will be available in October 2012. Completed applications will be due in January, 2013.
